This consultation gives us a chance to meet and discuss your concerns, goals and desired outcome, and to ensure both the patient and practitioner are happy to proceed with treatment.

 

Wear loose comfortable clothing as session does not require you to disrobe. This gives us time to discuss your case, go over necessary documentation, verify we have all necessary insurance information and time for a hands on assessment session.

 

Be sure to bring your doctors information, prescription, insurance card, and if this is an auto injury you also need to bring your claim number, adjusters contact, and lawyers information.

By working closely with your medical team this session is based on a course of treatment for rehabilitation of specific injuries. 

 

Currently in-network preferred provider for Blue Cross Blue Shield, Providence, and most auto insurance claims. When billing insurances, Medical Massage will be billed per unit. Any co-pays due are collected at the time of service. We offer a discount to patients who pay in full at the time of service.
